What It Is and Why It Happens
Achilles tendinopathy is an overloaded, painful Achilles tendon — the cord connecting the calf to the heel. It shows up as morning stiffness, pain at the start of a run or walk that eases with warm-up, a tender thickened area a few centimetres above the heel (mid-portion) or right at the heel bone (insertional). It is common in runners who increased volume, in people who stand and walk all day, and in anyone who changed footwear or started a new activity.
Like other tendinopathies it is not simply inflammation, so rest alone does not fix it; the tendon needs a graded loading program to rebuild its capacity.
Achilles and Calf Problems We Treat
Our physiotherapists at Cityscape Square NE assess and treat:
- Mid-portion Achilles tendinopathy
- Insertional Achilles tendinopathy and heel bursitis
- Achilles pain in runners and walkers
- Calf strains and tears
- Post-surgical Achilles repair rehab
- Achilles pain from safety boots and hard floors
- Sever’s disease (heel pain in active kids)
- Achilles stiffness after ankle injury
How We Treat Achilles Tendinopathy
Assessment identifies mid-portion versus insertional tendinopathy — they are loaded differently — and looks at calf strength, ankle mobility, foot mechanics and training or work load. Treatment is a progressive calf loading program (isometric, then heavy slow resistance), manual therapy and dry needling for the calf, heel lifts and footwear changes, and a return-to-running or return-to-work plan. Orthotics help when foot mechanics drive the load.
Sudden pain with a snap and inability to push off suggests a rupture and needs urgent assessment. See also plantar fasciitis and sports injuries.

Our Patients Share Their Journey
After my motor vehicle accident, I was dealing with intense neck pain, lower back discomfort, and stiffness that made even simple movements difficult. Coming to Apollo Physio truly made a huge difference in my recovery journey. I received treatment from Vidhi, my physiotherapist, Janki, my massage therapist, and chiropractic care from Dr. Dhaliwal and I couldn’t have asked for a better team. Vidhi created a well-structured treatment plan and guided me through exercises that gradually improved my strength and mobility. She explained everything clearly and kept track of my progress at every step. Janki’s massage therapy sessions were incredibly helpful in reducing muscle tightness and relieving pain. After each session, I felt more relaxed and could move more freely. Chiropractic sessions with Dr. Dhaliwal were also a key part of my recovery. His adjustments helped improve my alignment and significantly reduced the stiffness and discomfort I was experiencing. He was very professional, knowledgeable, and made sure I was comfortable throughout each session. Within a few weeks, I noticed major improvements less pain, better movement, and a return to my normal daily routine. The combined approach of physiotherapy, massage, and chiropractic care really made the recovery process faster and more effective. The clinic is clean, well organized, and has a very positive and welcoming environment. The front desk staff are friendly and always helpful with booking and insurance related questions. Usually not entirely. We reduce volume and intensity to a level the tendon tolerates and rebuild from there.
Achilles pain sits at the back of the heel and above; plantar fasciitis is under the heel. Both are common in the same people and both respond to loading.
