What It Is and Why It Happens
Frozen shoulder (adhesive capsulitis) is inflammation and thickening of the joint capsule that progressively restricts movement in every direction. It usually starts without an injury, is more common between 40 and 60, in women, and in people with diabetes or thyroid conditions, and it moves through three stages: a painful freezing stage, a stiff frozen stage, and a thawing stage where movement gradually returns. Left alone it can take one to three years.
Physiotherapy matched to the stage — pain control and gentle movement early, progressive mobilization and stretching later — reduces pain sooner and shortens the overall course.
Signs and Related Problems We Treat
Our physiotherapists at Cityscape Square NE assess and treat:
- Gradual loss of shoulder movement in all directions
- Deep aching shoulder pain, worse at night
- Inability to reach behind the back or overhead
- Pain with sudden movements or lying on the shoulder
- Stiffness after shoulder surgery or immobilization
- Frozen shoulder with diabetes or thyroid conditions
- Shoulder stiffness after a fracture
- Secondary neck and upper back pain
How We Treat Frozen Shoulder
Assessment confirms the diagnosis — true frozen shoulder restricts external rotation more than anything else — and identifies the stage. In the freezing stage treatment focuses on pain relief, gentle range of motion and sleep strategies; pushing hard here makes it worse. In the frozen and thawing stages we progress joint mobilization, capsular stretching, dry needling for guarded muscles and a graded home program. We coordinate with your physician if a cortisone injection would help you tolerate the mobility work.
Frozen shoulder that follows a collision or a work injury is covered under Section B or WCB. See also shoulder pain physiotherapy and rotator cuff injuries.

After my motor vehicle accident, I was dealing with intense neck pain, lower back discomfort, and stiffness that made even simple movements difficult. Coming to Apollo Physio truly made a huge difference in my recovery journey. I received treatment from Vidhi, my physiotherapist, Janki, my massage therapist, and chiropractic care from Dr. Dhaliwal and I couldn’t have asked for a better team. Vidhi created a well-structured treatment plan and guided me through exercises that gradually improved my strength and mobility. She explained everything clearly and kept track of my progress at every step. Janki’s massage therapy sessions were incredibly helpful in reducing muscle tightness and relieving pain. After each session, I felt more relaxed and could move more freely. Chiropractic sessions with Dr. Dhaliwal were also a key part of my recovery. His adjustments helped improve my alignment and significantly reduced the stiffness and discomfort I was experiencing. He was very professional, knowledgeable, and made sure I was comfortable throughout each session. Within a few weeks, I noticed major improvements less pain, better movement, and a return to my normal daily routine. No, especially in the freezing stage. Gentle movement within tolerance, progressed as the shoulder allows, is the approach.
